Walkthrough of the Registration Process

NOTE: If you want to have these instructions available to you while you register, you will need to either print them out or open www.ayso128.org in a new browser window or tab.

If you already have an account on the Region 128 website, then just log in. Otherwise, create an account on this site (www.ayso128.org), by clicking on the "Register Now” in the upper right corner of the page.

You will see the screen below open, where you can create a new account. Create the account with you own information.

After you have entered your information, click "Create Account". You will then see a page like the one below where you can fill in account details. Provide this information, and if you want to add an additional account holder, such as the other parent, you can do so at the bottom left of this page. Then click “Continue” at the bottom of the page.

You can now add participants. Fill in the information on this page (example shown below). You can register more than one participant at a time by using the button at the bottom left of the page. After you have added all the participant(s) information, click “Continue” at the bottom of the page.

Programs that are available to each participant will then be shown. For each participant, click the “+Register” button for the program you want to register them for, and then click “Continue”.

You will then be asked for more information about the participant(s) based on the program they are registering for (see example below).

There will be new uniforms this year for players in the 10U and above divisions. If your participant will be in one of these programs, please be sure to indicate uniform size, and check the “new uniform” box near the bottom of the page. Then click “Continue”.

The next page will request that you start the electronic signature process by clicking the “Click Here to eSign Form” button.

The next page will present the registration form, filled out for your player. Be sure to read the instructions at the top of the page.

Midway down the page, you will need to check the box agreeing to eSign. Then you can click the button indicating parent (or click self if you are playing and are 18 years old), and type your name in the appropriate box.

Then scroll to the bottom of the page and click “Continue to Review”.

You will again see the registration form, this time with your ‘signature’ included. Please review the form, and at the bottom of the page click “Submit signed player registration”.

The e-Signature process should now be complete. Click the “Continue” button on the next page.

The next page (example below) will present volunteer positions that are available. You can sign up for a position at this time, or you can return and sign up later. Whenever you do sign up, only volunteer / sign up for one position initially, otherwise you will have to fill out duplicate forms with the same information. If you want to volunteer for more than one position, register for one position at first, log out, then return and register for the other position(s). That way all of your information should be pre-filled for you the second time you register.

At the bottom of the page, click “Continue”.

You will then be presented with a registration summary. Look to be sure your cart is correct, and then click the blue “Continue” button. This will open up the payment information portion of the screen. Fill it in, and after this is complete, again click the blue “Continue” button. The Confirmation section of the screen will then open. Review the information, check the “I agree to the terms and conditions” check box, and then click “Continue”.
